Kindergarten Registration

Registration of children who will reach five years of age by October 1 of this year was held at Frank J. Smith School (27 Green Drive) in February. If you're new to town, please contact the Board Of Education, and be prepared to bring the following documents:

bullet

Original birth certificate

bullet Evidence of at least four doses of DPT vaccine including one on or after the fourth birthday
bullet Evidence of at least three doses of oral polio vaccine including one after the fourth birthday
bullet Evidence of one dose of measles vaccine on or after the first birthday and a second dose of measles vaccine, preferably MMR; laboratory documentation of measles immunity is an acceptable alternative
bullet Evidence of one dose of German measles (rubella) vaccine on or after the first birthday; laboratory documentation of rubella immunity is an acceptable alternative
bullet Evidence of one dose of mumps vaccine on or after the first birthday; acceptable alternatives are a doctors certification of having had mumps or laboratory evidence of mumps immunity
bullet Evidence of three doses of hepatitis B vaccine
bullet Evidence of one dose of chicken pox varicella) vaccine on or before the first birthday; laboratory evidence of immunity or  doctor's or parent's statement of previous chicken pox infection are acceptable alternatives
bullet Three proofs of East Hanover residence such as a utility bill, tax bill, etc.

Additionally, children must have a physical exam within a year before entering Kindergarten; medical forms are available at Frank J. Smith School.

Parents are encouraged to register even if all of the documentation above isn't available, however the documentation must be provided before the child can begin attending school.
